Doctor Visit Cards
As you may be aware in 2023 the government announced the expansion of free GP care to all children under the age of 8 and and to households earning below the new median income threshold.
Eligible current practice patients can now apply as follows:
-
If your child is aged 6 or 7, you can apply online for a Doctor Visit Card.
-
If your household earns below the median income (€47,000 per year after deductibles) you can apply online for a Doctor Visit Card.
Please direct your request to Dr. Louise Freeman's Panel.

About Lakelands Medical Centre
Lakelands Medical Practice has grown from a single handed practice set up by Dr. Joseph Curran in Clonbur, Co. Galway in 1996. Dr. Louise Freeman joined the Practice in 2020 and in 2021 Dr. Joe and Dr. Louise formed a partnership and opened a second surgery in Ballinrobe. We are fortunate to have Dr Conor Farrell and Dr. Aliyah Lockhat working with us.
We are supported by a team of fantastic nurses who provide a wide range of services and administrative staff who keep the whole show running day to day.
Our surgery is wheelchair accessible and parking is available directly outside the surgery.
We offer a wide variety of medical services to medical card and private patients including antenatal and postnatal care, contraception services, childhood vaccinations, 24 hour BP monitors, menopause clinics, spinal manipulation, minor surgery, and well man/well woman annual check-ups. We also provide clinics for the management of chronic diseases such as cardiovascular disease, diabetes, asthma and COPD.
Our aim is to provide high quality, evidence based, patient centred care. We want to engage and empower you to achieve the very best health outcomes for you. We are an LGBT+ friendly practice.
We look forward to welcoming you to Lakelands Medical Practice.
